Here in Ireland we have had huge problems with BYOD, employees not securing their laptops and mobile devices with the basics, like passwords. Another problem is physical theft - laptops being stolen from CFOs and Directors of large Insurance and Financial firms, with un-secured information like account details and financial information about customers - so it happens to the best of them.
I believe it is the responsibility of everyone - the firm and the employee/director to protect their devices/information as BEST they can.
Of course with the onset of cloud computing, this is posing greater problems for the smaller firms, and the more traditional firms, reluctant to move to the cloud. Education and awareness are key.
